0 DATURA PIOSONING AND MANAGEMENT




v  Datura plant is a poisonous plant whose all parts are poisonous but fruit and seeds are most toxic.
v  It produces anticholinergic  syndrome
v  Its scientific name is “datura stramonium “ (jimson seed).
v  Mode of poisoning is by mixing the seed in food items and giving to other.
v  Atropine & scopolamine are the toxin agent present in it.

Clinical features or symptoms

v  Symptoms appear within 30 minutes of ingestion and last for 1 to 2 days.
v  Classical symptoms includes feeling of extreme hot (hot as a hare ) ,blinds ness ( ciliary muscle paralysis),dryness(dryness of mouth ,urinary retention ,bowel movement reduced).
v  Redness (cutaneous vasodilatation causing flushing ), & madness(CNS arousal, delirium & hallucinations).
v  The pupils become dilated.
v  Stupor, coma& convulsions occur in the fatal cases.
v  Death occurs due to respiratory failure or cardiovascular collapse.

management

v  Gastric emptying is done immediately.
v  Supportive is given to all the vital organs.
v  Hyperthermia is controlled.
v  Urinary catheterisation is done.
v  Agitation are controlled by the use of benzodiazepines.
v  Some use physostigmine but its use is controversial.

0 OLEANDER POISONING & MANAGEMENT



Ø  Oleanders you almost have seen it but have never thought of it. Being a medico what I knew is that every particle is a poison until and unless it exceeds it normal limit.
Ø  Yes oleanders are also poisonous and are fatal if consumed a little more.
Ø  Usually not all oleanders are poisonous, only yellow oleander is fatal. It is also known as thevetia  Peruvian.
Ø  The oleander plant contains glycosides which resemble digitoxin, a cardiac glycoside.
Ø  Hence it poisonous effects mainly over the heart.

Symptoms or clinical features

v  Within 3 hours the symptoms start causing mainly gastro intestinal symptoms.
v  Symptoms include: nausea, vomiting, diarrhoea and abdominal pains.
v  Others symptoms belong to cardiac origin which includes: heart rates reduce, and the patient may become anxious.
v  Some other features include: dizziness, numbness, tingling, restlessness, hypotension and also abdominal cramps may also occur.

Lab investigations

v  Hyperkalaemia and acidosis develops.
v  Hyperkalaemia means increased concentration of free potassium responsible for cardiac symptoms.
v   Digoxin is estimated by radioimmunoassay.

treatment

*      Management of the oleander poisoning mainly includes supportive management as there is no any antidote available.
*      Fluid, electrolyte balance & acid base disturbances need to be corrected.
*      Gastric emptying is done immediately.
*      Activated charcoal is given repeatedly.
*      Atropine corrects all the cardiac defects including bradycardia & arrhythmias.
*      If ventricular tachy- arrhythmias are present then a drug named lidocaine is given.
*      If the poisoning is very severe then digoxin specific Fab antibody fragments is provided.

1 Mushroom poisoning & its management



v  Only a few types of mushrooms are poisonous among the many types.
v  If the onset symptoms occur within 3 hours of ingestion of mushroom, then most likely it’s not deadly mushroom so you can be relax.
v  “Amanita phalloides” is one of the mushrooms responsible for majority of deaths. Also called as death cab.
v  The two types of poisons which it contains is “ phallotoxins” and “amatoxins”.
v  The former cause’s gastritis while later is responsible for liver & renal damage.


Ø  Let us tell you the fact that most cases are due to unknowingly ingesting an unknown poisonous mushroom so be careful eating any unknown mushroom type of diet.

Management

Ø  Alternatively if it is not available give silymarin.
Ø  Both these reduce uptake of the drug by the liver. Hence the person can be saved from death.

Ø  They only cause kidney and liver failure hence management is only supportive.
